Ages

Well child appointments should be scheduled at the following intervals. You may complete the well child questionnaire corresponding to your child’s age prior to your appointment.

  • Under 2 years old: Newborn, 1 mo., 2 mo., 4 mo., 6 mo., 9 mo., 12 mo., 15 mo., 18 mo.
  • 2-21 years old: Annually

Developmental screening

Well child checks help us keep track of important milestones in your child’s development. Some well visits will include one or more brief questionnaires. These screening tools will help us see whether your child is at risk for problems, now or in the future. With this information, we can make a plan on what to do next. You will also be able to talk to your childā€™s provider about any concerns you have.

  • ASQ – (Ages & Stages Questionnaire)
    9, 18, & 24-months. Identify risks for developmental delays and behavioral problems. These forms will be provided at your office appointment.
  • MCHAT-R/F – (Modified Checklist for Autism in Toddlers)
    18 & 24-months. Please complete form prior to your appointment.
  • ADHD – (Attention Deficit Disorder Screening)
    6-12 years old. Please complete the ADHD Vanderbilt Parent and Teacher assessment forms prior to your appointment.
  • PSC-35 – (Pediatric Symptom Checklist)
    4-11 years old. Please complete form prior to your appointment.
  • SCARED – (Screen for Child Anxiety Related Disorders)
    8-18 years old. Please complete form prior to your appointment.
  • PHQ-9 – (Patient Health Questionnaire)
    12-18 years old. Please complete form prior to your appointment.

Most insurers will pay for this screening service. If your insurance denies this claim, you will be responsible for payment. A discount is available if paid on the date of service. Please be assured we are working with the American Academy of Pediatrics to make sure this service will become fully covered by all insurers and Medicaid. If you do receive a bill for developmental-behavioral screening after denial of the claim by your insurer, please understand how important this service is to your childā€™s well-being.

Immunizations

Staying up to date on immunizations protects your child from many harmful diseases. We follow the CDC’s schedule to best prevent these illnesses. Your child’s health is our top priority. Our providers are happy to discuss any questions or concerns you may have. View immunization schedule.

Sports physicals

A sports physical is required to participate in school sports. Annual well checks meet this requirement. These appointments are the perfect time to complete the form for your child’s school.
