1. Create your own MyChart account
To access your childās MyChart account, you will need to set up your proxy access. This means you, the parent or guardian, must have your own MyChart account first. (Please note: foster parents cannot have proxy access to their foster child’s MyChart account.)
There are three easy ways to create a MyChart account:
- Online: Create a new account. After you submit your registration information, it will take 1-2 business days for processing and verification. Once approved, you will receive an email with your activation code and instructions on how to activate your MyChart account.
- Over the phone: Call 253-383-5777. Our staff can guide you through the registration process over the phone and mail your activation code right to your home. We can also enable your childās chart for proxy access, negating step three below.
- In office: Our staff can guide you through the registration process while you are in the office and print your activation code. No appointment is necessary and you can go to any of our office locations. We can also enable your childās chart for proxy access, negating step three below.
2. Activate your account
Once you have received an activation code, you will need toĀ Activate Your AccountĀ online by entering the MyChart activation code exactly as it appears on your āWelcome to MyChart Letterā.
3. Enable your proxy access
Once you have your own MyChart account, you need to receive an Activation Code from our office to enable your childās chart for proxy access.Ā You will not be able to access your child’s account until you receive the proper Activation Code.
If you set up your account online, you will need to stop by our front desk or give us a call so we can set up your proxy access.
If you set up your MyChart account in person or over the phone, this has already been completed. You will now access your childās MyChart through your own MyChart access.
